Hypersensitivity pneumonitis (HP) is an immune-mediated interstitial lung disease driven by repeated antigen exposure in susceptible individuals. Cough is a prominent but poorly characterized symptom, especially in relationship to objective measures and treatment response.

Patients with interstitial lung disease-related connective tissue disease (CTD-ILD) may demonstrate pulmonary hypertension (PH) through pulmonary parenchymal destruction and chronic hypoxia or via endovascular changes driven by the underlying autoimmunity. Distinguishing which process-parenchymal or vascular-is the primary cause has important therapeutic and prognostic implications.

Transbronchial cryobiopsy has expanded beyond its established role in interstitial lung disease, driven by release of single-use cryoprobes, most notably the 1.1 mm cryoprobe. As use of transbronchial cryobiopsy accelerates across non-interstitial lung disease diagnostic contexts, practice variability has outpaced evidence generation and formal guidance.
